Since 2001, CHC has supported local health programs, policies, environmental and system changes to achieve healthy communities in 24 Ohio counties. These 24 projects address the risk factors of poor nutrition, tobacco use and exposure, sedentary lifestyle and chronic disease management in the communities’ schools, worksites and healthcare settings. These outstanding initiatives impact the lives of 3,877,072 at-risk Ohioans.
We would like to congratulate the Ohio Department of Health’s Creating Healthy Communities (CHC) Program for being the recipient of the Preventive Health and Health Services Block Grant Champion Award.
